To complete your enrollment, please select one of the forms below.  Once you download and complete the applicable form for your child, please email it to inspire.artcamp@gmail.com 48 hours before your child's first class.

Studio Enrollment Form 

(For in-studio programs)

(For programs at your child's school)

How to complete and return your Enrollment Form:

Please download and complete the attached fillable PDF document in full, sign and save it.  All sections pertaining to your enrolled child/family are required.   If you complete the date field on top of the page, the rest of the date fields will be automatically completed for you.  The same applies to your name and signature fields. Please check all boxes/fields that apply.  Once you have saved your form, please email it to us.  

Please note:  Forms can be completed on any device, but we CANNOT accept forms completed using Google Docs.
